Some of the most common side effects include:
- Tiredness or fatigue – especially when first starting the medication or after a dose increase.
- Dizziness or lightheadedness – often due to lowered blood pressure, particularly when standing up quickly.
- Slow heart rate (bradycardia) – metoprolol is designed to slow the heart, but sometimes it can slow it too much.
- Cold hands and feet – because blood flow to the extremities may be reduced.
- Low blood pressure – which can cause weakness or fainting.
- Shortness of breath or reduced exercise tolerance – more likely in people with asthma or other lung conditions, though metoprolol is generally less likely to affect the lungs than some other beta blockers.
- Digestive symptoms – such as nausea, diarrhea, constipation, or stomach discomfort.
- Sleep disturbances – including insomnia or vivid dreams in some people.
- Headache – usually mild and often temporary.
- Mood changes – some people report depression or changes in mood, though this is less common.
Seek medical attention promptly if you experience:
- Fainting or severe dizziness.
- A very slow heart rate or new irregular heartbeat.
- Chest pain that is new or worsening.
- Difficulty breathing, wheezing, or swelling of the face or throat.
- Signs of an allergic reaction.
Important
- Do not stop metoprolol suddenly. Abruptly stopping it can lead to rebound high blood pressure, chest pain, or, in some cases, a heart attack. If it needs to be discontinued, your healthcare provider will usually recommend tapering the dose gradually.
