Lou Gehrig’s Disease (ALS): Early Symptoms to Recognize

Lou Gehrig’s disease, also known as amyotrophic lateral sclerosis (ALS), is a progressive neurological condition that affects the nerve cells responsible for controlling voluntary muscles. Early symptoms can be subtle and may differ from person to person.

Possible first symptoms include:

1. Muscle weakness

  • Weakness in one hand, arm, leg, or the muscles used for speaking may be an early sign.
  • People may notice difficulty gripping objects, climbing stairs, or performing everyday tasks.

2. Muscle twitching (fasciculations)

  • Small, involuntary muscle twitches may occur, often in the arms, legs, shoulders, or tongue.

3. Muscle cramps and stiffness

  • Some people experience frequent cramps, tightness, or stiffness that does not improve.

4. Tripping or changes in walking

  • Foot weakness or reduced coordination may cause stumbling or difficulty walking.

5. Changes in speech or voice

  • Slurred speech, a softer voice, or trouble forming words can occur in some cases.

6. Difficulty swallowing

  • Problems moving food or liquids safely can develop as muscles involved in swallowing are affected.

7. Loss of fine motor skills

  • Tasks such as writing, using buttons, or handling small objects may become harder.

8. Muscle fatigue

  • Muscles may tire more quickly during activities that were previously easy.

Important:

These symptoms do not automatically mean someone has ALS. Many other conditions—such as nerve compression, vitamin deficiencies, thyroid problems, injuries, or other neurological disorders—can cause similar symptoms.

A medical evaluation is important if someone has progressive muscle weakness, persistent twitching combined with weakness, speech changes, or swallowing difficulties. A neurologist can perform the appropriate tests to determine the cause.
